Referral To Acute Rehabilitation

Upon physician request, the Intake Development Coordinator will evaluate your rehabilitation needs and address the requirements for admission. This includes coordinating insurance verification and providing you and your family with information about the hospital.

Treatment Plan

Specialty Rehabiliation Hospital of Luling offers a caring and safe environment in which patients and their caregivers learn to manage impairments affecting self-care, mobility, communication, cognitive skills, and other activities of daily living. A multidisciplinary team of physicians, nurses, social workers and therapists assess the needs of the patient and establish the treatment plan and goals. A daily treatment schedule is set up based on the individual needs and goals, and practiced throughout the day with all members of the team.

Discharge Planning

Discharge planning begins at admission, and the case manager or social worker will assist you and your family with all discharge needs, including ordering of equipment and providing referrals to home care, outpatient therapy or community services.
